Job Description

Provides clinical pharmaceutical care services using approved protocols in collaboration with other health professionals, including managing patient drug therapy, providing patient education, and preparing and/or overseeing the preparation and dispensing of medications. Promotes high quality, evidence-based, cost-effective pharmaceutical care. Performs these duties in the clinical areas assigned.

Essential Responsibilities

Upholds Kaiser Permanentes Policies and Procedures, Principles of Responsibility, and all applicable federal, state, and local laws and regulations.

Conducts comprehensive patient medication history reviews upon referral from physicians and other healthcare professionals.

Synthesizes patient-specific treatment plans based on diagnosis, including pharmacologic therapy and lifestyle modifications, and facilitates care transitions across the continuum of care.

Identifies, resolves, and proactively prevents actual and potential medication-related problems, including untreated indications, inappropriate drug selection, subtherapeutic dosing, overdose, medication non-adherence, adverse drug reactions, drug interactions, and unnecessary therapy.

Initiates, adjusts, monitors, and recommends modifications to drug therapy in accordance with established protocols, using information obtained from patient interviews, clinical parameters, medication profiles, and medical records.

Coordinates, orders (as applicable), and interprets laboratory tests related to drug therapy, including serum drug levels, electrolytes, and other relevant diagnostics.

Monitors patient outcomes to optimize therapeutic effectiveness, medication utilization, and adherence; identifies and mitigates adverse drug reactions; and eliminates unnecessary or duplicative therapy.

Provides patient-centered education regarding disease states, medication regimens, expected therapeutic effects, potential side effects, and adherence requirements.

  • Ensures continuous quality and performance improvement of pharmaceutical care services.
  • Conducts drug utilization reviews, comprehensive medication reviews, and medication reconciliation in accordance with organizational standards
  • Operates pharmacy information systems, dispensing technology, and other relevant equipment and software.

Documents clinical activities and interventions accurately and timely in the patient medical record and applicable systems in compliance with policies, procedures, and regulatory requirements.

Provides oversight, guidance, and direction to ancillary and support staff, including pharmacy technicians, assistants, coordinators, and interns.

Performs other duties as assigned or required to support departmental and organizational objectives.

Basic Qualifications

Experience

Minimum one (1) year of experience in a clinical pharmacy setting, or successful completion of a clinical pharmacy (PGY-1) residency or fellowship.

Education

Bachelors or Doctorate of Pharmacy (Pharm. D).

  • License, Certification, Registration
  • Pharmacist License (Nevada) required at hire
  • National Provider Identifier required at hire
  • Basic Life Support within 3 months of hire

Additional Requirements

Completion of a standardized competency assessment, as applicable (excluding current Ambulatory Care Pharmacists).

  • Knowledge of pharmacological principles, disease states, medical terminology, and age-appropriate therapy.
  • Working knowledge of applicable federal and state pharmacy laws and regulations.
  • Knowledge of aseptic technique and preparation of sterile and hazardous medications, as required.
  • Familiarity with drug information resources and medical literature.
  • Ability to gather and interpret relevant patient and clinical information to support care decisions.
  • Ability to effectively educate patients, staff, and health care providers.
  • Strong verbal and written communication skills.
  • Demonstrated analytical and problem solving skills.
  • Experience working collaboratively within a multidisciplinary patient care team.

Preferred Qualifications

  • Doctorate of Pharmacy (Pharm. D)
  • PGY-2 clinical pharmacy specialty residency or fellowship
  • Advanced Practice Pharmacist License
