In Your Role You will provide comprehensive, longitudinal care for patients of all ages.

Your responsibilities will include: Diagnosing and managing acute and chronic conditions Conducting annual wellness visits and routine physical exams Managing chronic diseases such as diabetes, hypertension, high cholesterol, and obesity Administering age-appropriate immunizations and preventive screenings Performing well-woman exams, including birth control counseling Educating patients on health maintenance, lifestyle changes, and disease prevention Collaborating with specialists and care teams to ensure coordinated, holistic care
