The ED RN Case Manager monitors real-time ADT notifications to provide proactive patient outreach and discharge coordination. They collaborate with hospital teams and outpatient resources to ensure optimal transitions of care and continuity for members.
Candidates must hold an undergraduate degree in Nursing and a current, unrestricted Multistate Registered Nurse (RN) license. A minimum of 4 years of diverse clinical experience with acutely ill patients is required.
