Physical Therapist (PT) – Home Health

Key Responsibilities

The Physical Therapist will conduct in-home evaluations and develop individualized treatment plans to improve patient mobility, strength, and function. They are also responsible for educating patients and caregivers while maintaining accurate clinical documentation.

Requirements

Candidates must hold an active Florida Physical Therapist license and possess strong assessment and treatment skills. Previous home health experience is preferred, and the ability to work independently is required.
