Key Responsibilities
- Provide direct, evidence-based primary care to patients with cancer, both the office and virtually.
- Maintain close collaboration with the partner oncology providers who are managing the patient’s cancer.
- Coordinate care and optimize referrals to established primary care and specialty care providers in the community.
- Participate in the delivery of specialized clinical programs, including congestive heart failure, frailty, and behavioral health.
Requirements
- You hold an unrestricted MD or DO state license, including board certification for Internal Medicine or Family Medicine valid in the state of practice.
- At least 4 years of direct patient care experience in primary care.
- Extensive knowledge and experience in areas relevant chronic disease management, and palliative care.
- Strong ability to collaborate with diverse teams, build relationships, and foster a positive work environment.

- Part-Time position.
