Job Description
The Adult Day Care Center located @ Waters Place in the Bronx, is seeking a full time Social Worker to join our team!
Job Responsibilities
- Work as part of an interdisciplinary team to provide comprehensive MDS and psychosocial assessments for clients in our Dementia Care our Enhanced Memory Impaired program.
- Coordinate and facilitate care planning and completion of all documentation
- Facilitate team and family meetings, education on older adult's rights and well-being, needs of the vulnerable elderly population. This involves extensive work with family members and friends.
Key Responsibilities
Conduct comprehensive MDS and psychosocial assessments for clients in dementia care and the Enhanced Memory Impaired program as part of an interdisciplinary team. Coordinate care planning and documentation, facilitate team and family meetings, and provide education on older adults’ rights, well-being, and the needs of vulnerable older adults.
