Key Responsibilities

Provide high-quality, patient-centered physical therapy services in patients' homes by evaluating mobility and developing individualized treatment plans. Collaborate with a multidisciplinary healthcare team to monitor patient progress and educate families on therapy techniques and safety.

Requirements

Candidates must be graduates of an accredited Physical Therapy program and hold a current Connecticut Physical Therapist license. At least one year of practicing experience is required, with preference given to those with a background in home health.
