We’re currently hiring for a Part-time Physical Therapist to join our collaborative interdisciplinary team and provide care across a variety of settings, including independent living, assisted living, memory support, and skilled nursing.

Qualifications

Job Summary: The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.

Qualifications

Bachelor's, Master’s or Doctorate degree in physical therapy from an accredited program. Certification and licensure requirements as established by state and national board associations. Active state license required.

Excellent organizational and time management skills Excellent written and verbal communication skills Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.
