What You'll Do
As a Hospice Social Worker, you will:
Conduct Comprehensive Assessments: Evaluate patients' social, emotional, and environmental needs to develop personalized care plans.
Provide Counseling and Support: Offer emotional support and counseling to patients and their families, addressing end-of-life issues and facilitating coping strategies
Collaborate with Interdisciplinary Teams: Work alongside physicians, nurses, and spiritual care providers to ensure holistic care delivery.
Advocate for Patients: Ensure patients' needs and preferences are respected, acting as a liaison between patients, families, and healthcare providers.
Coordinate Community Resources: Assist patients and families in accessing necessary care and benefits by connecting them with community resources.
Maintain Accurate Documentation: Keep detailed records of patient interactions and progress in compliance with regulatory standards.
What You Need
- A Master's degree in Social Work (MSW) from an accredited institution is preferred.
- Must be a licensed social worker(LSW OR LCSW) in New Jersey.
- Minimum of two years in clinical settings, preferably in hospice, palliative care, or related fields.
Strong interpersonal and communication abilities, with a deep understanding of grief, loss, and the emotional aspects of end-of-life care.

The Hospice Social Worker will conduct comprehensive assessments to develop personalized care plans and provide emotional support to patients and their families. They will also collaborate with interdisciplinary teams and coordinate community resources to ensure holistic care delivery.
Requirements
Candidates must hold a Master's degree in Social Work and be a licensed social worker (LSW or LCSW) in New Jersey. A minimum of two years of clinical experience in hospice or palliative care is required.
