Physical Therapist PT Home Health (PRN)

Empath Home Health, a member of Empath Health is seeking a compassionate PRN Home Health PT Physical Therapist to work with us to provide exceptional and individualized care in Manatee county (Bradenton, Lakewood Ranch, and surrounding areas).

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations: Home health visits in Manatee county (including Bradenton, Lakewood Ranch, Anna Maria Island, Ellenton, Palmetto, and surrounding areas).

Visits may also be available in Sarasota and/or Hillsborough counties if desired.

Schedule

PRN; 5-10+ home health visits per week. Required weekend/holiday on-call rotations.

What You'll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled physical therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

Active Florida PT License

• Minimum of 1 year experience as a licensed Physical Therapist (PT).
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday rotations
