Key Responsibilities
Collaborates with the interdisciplinary team to manage patient discharge planning and ensure access to community resources. Provides crisis intervention, psychosocial assessments, and ongoing support for unhoused patients to facilitate safe transitions and long-term stability.
Requirements
Requires a Master of Social Work (MSW) degree and the ability to obtain SPDAT training within six months of hire. Preferred candidates have experience working with vulnerable populations, including those with mental health conditions and complex medical needs.
