Key Responsibilities
Provide direct clinical care to patients with neurological and memory disorders via telehealth. Collaborate with physicians to develop care plans and maintain accurate clinical documentation in the EMR.
Requirements
Requires an active, unrestricted Nurse Practitioner license in Connecticut and at least 2 years of clinical experience in neurology or memory care. Candidates must be comfortable managing a patient panel independently and possess strong clinical documentation skills.
