Speech-Language Pathologist – Telehealth/Per Diem

Bakersfield Post Acute

Pay Range: $60.00–$65.00 per hour, based on experience

Bakersfield Post Acute is seeking a compassionate and experienced Speech-Language Pathologist (SLP) to join our rehabilitation team in a telehealth/per-diem capacity. This position evaluates and treats residents with speech, language, cognitive-communication, and swallowing disorders while collaborating closely with residents, families, physicians, nursing staff, and the interdisciplinary care team.

The ideal candidate has skilled nursing experience, excellent clinical judgment, and the technology skills needed to provide engaging, resident-centered telehealth services.

Essential Duties and Responsibilities

Conduct comprehensive evaluations to determine residents’ speech, language, cognitive-communication, voice, and swallowing needs.

Assess functional limitations and rehabilitation potential in accordance with physician orders and facility policies.

  • Develop, implement, and update individualized plans of care with measurable goals.
  • Provide medically necessary treatment through an approved telehealth platform.
  • Monitor resident progress and modify treatment plans when appropriate.

Educate residents, families, and caregivers regarding treatment goals, compensatory strategies, maintenance programs, and aspiration precautions.

Communicate regularly with physicians, nursing staff, rehabilitation professionals, residents, and families regarding progress, goals, and discharge planning.

Participate remotely in resident-care conferences, utilization-review meetings, and rehabilitation meetings as needed.

Complete timely, accurate, and compliant evaluations, progress notes, treatment documentation, and discharge summaries.

  • Follow facility, governmental, and third-party payer documentation requirements.
  • Coordinate with on-site staff to promote resident safety and the successful delivery of telehealth services.
  • Provide staff education regarding communication, cognition, swallowing, and aspiration-prevention strategies.
  • Assist with training Restorative Aides when appropriate.
  • Recommend adaptive equipment and strategies that support safe swallowing, communication, cognition, and independence.
  • Contribute to quality-assurance, performance-improvement, and rehabilitation-program initiatives.
  • Supervise Clinical Fellows or students when applicable and permitted by state requirements.
  • Maintain resident privacy and confidentiality throughout all telehealth interactions.
  • Perform additional duties within the SLP’s scope of practice as assigned.

Qualifications

  • Master’s or doctoral degree in Speech-Language Pathology from an accredited program required.
  • Current California Speech-Language Pathologist license in good standing required.
  • Current ASHA Certificate of Clinical Competence preferred.
  • Skilled nursing, long-term care, geriatric, or post-acute rehabilitation experience preferred.
  • Experience evaluating and treating dysphagia, cognitive-communication disorders, and speech or language impairments preferred.
  • Telehealth experience preferred.
  • Proficiency with electronic documentation systems such as Net Health and PointClickCare preferred.
  • Ability to use videoconferencing technology and provide effective remote treatment.
  • Strong clinical reasoning, organization, and communication skills.
  • Current CPR certification if required by licensure or facility policy.
  • Must satisfy all continuing-education and professional requirements.

Supervisory Responsibilities

  • May supervise Speech-Language Pathology Clinical Fellows or students when applicable, qualified, and permitted under state requirements.
