Job Description
- ability weekly, consistently providing 320+ capacity units per month Execute a strong visit completion rate, consistently pacing 220+ completed visits per month Maintain training, affiliation, and compliance with Video Visit requirements (regular training required) Other role expectations and requirements: Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s) Consistently engage with regular manager, team and clinical meetings Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics Maintain active state licenses in coverage areas (FNP, AGNP certification preferred for NPs) Maintain board certification Primary coverage areas include but are not limited to: Nebraska Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format (preferred) Able to obtain and maintain multi-state licenses in coverage area, as required Additional Required Skills, Abilities and Working Environment: Clinicians will be dedicated to providing Video Visits (“telehealth”) services only Must meet minimum requirements for remote visit delivery, including broadband connectivity with video and audio enabled, a quiet setting with a neutral background from which to conduct visits, and the ability to uphold health plan member privacy per CVS Health guidelines. Key Responsibilities
Conduct 30-minute virtual video visits with health plan members to review medical histories and medications. Educate members on their health and help close gaps in care to connect them with appropriate medical services.
Requirements
Must be a board-certified Nurse Practitioner or Physician Assistant with an active state license in Nebraska. Requires a private workspace with high-speed broadband connectivity and the ability to maintain clinical compliance metrics.
