Social Worker 2 - Oncology

Key Responsibilities

The Social Worker provides psychosocial assessments and interventions to oncology patients while coordinating efficient discharge planning within a multidisciplinary team. They also serve as a resource for staff, assist with staff orientation, and facilitate linkages to community resources to support patient care goals.

Requirements

Candidates must hold a Master's Degree in Social Work and possess a Licensed Social Worker (LSW) certification. Previous experience in a hospital or health agency setting is preferred, with a strong preference for post-master's experience in oncology or chronic disease management.
