Key Responsibilities
The social worker will provide psychosocial assessments and interventions to oncology patients while coordinating discharge planning and community resource linkages. They will also collaborate with an interdisciplinary team to support patient transitions and manage complex psychosocial barriers to care.
Requirements
Candidates must hold a Master's degree in Social Work and possess a Licensed Social Worker (LSW) certification. Previous experience in a hospital or health agency setting is preferred, along with proficiency in clinical health care delivery.
