Key Responsibilities

The Medical Social Worker conducts psychosocial assessments, develops care plans, and facilitates discharge planning to ensure safe transitions for patients. They collaborate with a multi-disciplinary team to manage utilization, coordinate community resources, and advocate for patient needs throughout their hospital stay.

Requirements

Candidates must hold a Master's degree in Social Work from a CSWE-accredited school and possess at least 2 years of healthcare social work experience. Licensure or eligibility for LSW/LCSW in Illinois and Wisconsin is required.
