The Transition Counselor supports American Indian students with disabilities in transitioning from high school to employment or post-secondary education. This includes developing Individualized Plans for Employment (IPE) and collaborating with educational institutions and community agencies.
A Bachelor's degree in Rehabilitation, Psychology, Sociology, or a related field is desired, or equivalent experience in social services or case management. Candidates must have a valid driver's license and the ability to pass a background check and drug test.
