Provide skilled hospice care to adult and geriatric patients in homes, assisted living, and inpatient facilities. Collaborate with an interdisciplinary team to promote comfort, safety, and mobility for patients at the end of life.
Requires a license as a Physical Therapist and a degree from an accredited program with at least one year of clinical experience. A valid driver's license and reliable transportation are mandatory for field-based visits.
