Job Details: Job Location: Port Gamble S'Klallam Tribe - Kingston, WA, Position Type: Full Time - 30 - 40 hrs/week, Education Level: Masters Degree - Required, Salary Range: $31.12 - $42.01Hourly, Travel Percentage: Occasional travel to trainings, The Behavioral Health Consultant (BHC) specializes in brief, evidence-based, solution-focused assessments and interventions geared toward functional improvement.
BHCs are generalists, providing high-volume services from a population-based lens that are accessible, team-based, and a routine part of primary care. They will Identify, triage, and co-manage patients with medical and behavioral health problems within the primary care setting.
They will provide skill training through psychoeducation and patient education strategies and will develop specific behavioral change plans for patients and behavioral health protocols for target populations.
They will support both the primary care providers and the patients by providing quality clinical mental health consultations to providers and patients as requested. The Behavioral Health Consultant is a member of the Integrated Clinical Care team providing patient-centered care.
Duties
Include: Assess each patient's mental health and developmental needs and create a treatment plan to reduce negative behavior and promote positive choices Provides quality clinical consultation to providers and staff in areas of individual, family, child, and adolescent regarding mental and behavioral health-related issues that are directly connected to the patient’s medical condition.
Administer, review, and respond to brief patient screenings, using the PGST general screening tool to identify patient needs. Provide brief (10-20 min) solution-focused behavioral interventions with active development of behavioral change planning for patients referred to Behavioral Health via PGST Care Providers.
Provides follow-up appointments to deliver psychotherapeutic care related to the mind/body connection with medical problem presentations. Cultivates education, administration, and self-management skills for targeted populations with psychological or behavioral health issues.
Promotes the primary care provider's knowledge of patient behavioral health issues and provides education to enhance primary care utilization of behavioral health interventions. Teach patients self-care. Teach families and health center staff techniques for patient care.
Teach patients and health center staff techniques for preventing risks to the patient's well-being and techniques for enhancing the patient's treatment. Promotes a smooth interface between medicine and specialized mental/behavioral health care throughout the organization.
Collaborates and co-manages the patient’s behavioral health care with consistent, expeditious feedback to referring providers. Provides on-site availability for crisis triage and intervention.
Documents the clinical behavioral health aspect of patient interventions and tracks the patient referrals to specialty behavioral health care. Completes and submits all encounter documentation for billing purposes promptly.
Function effectively in a fast-paced, highly productive setting, with the ability to successfully pivot from a patient-facing to a physician-facing role throughout the day.
Reporting is mandatory for tribal employees and contractors with the tribe who perform services to the community in the areas of education, health and human services, law enforcement and members of the clergy.
Education and Certifications: A master’s degree in the behavioral health field, including Counseling, Psychology, or Social Work; Licensed Mental Health Counselor, Clinical Social Worker, or related license in the State of Washington. Washington State Driver’s License is required.
Experience: 3 years of specialized experience (with AI/AN patients, preferably) is in or related to the line of work of the position, which has provided the applicant with specific knowledge, skills, and abilities to perform the job duties successfully. Experience using electronic health record systems is required.
Business computer skills, e. g., email, word processing, presentations, and spreadsheets, are required. Physical Requirements: Regularly required to finger, handle, reach with hands and arms Frequently required to walk, sit, stand. Occasionally required to lift and move up to 25 pounds.
Normal audio and visual acuity Travel Requirements: Local, Statewide, and out-of-state travel may be required.
