Key Responsibilities
The Social Worker plans and implements social work services, conducting psychosocial assessments and acting as a liaison between participants and the interdisciplinary team. They also manage care coordination, including discharge planning, end-of-life support, and ensuring compliance with regulatory requirements.
Requirements
Candidates must hold a Master’s degree in Social Work from an accredited school and possess at least one year of experience in a healthcare setting. A valid driver’s license, reliable personal transportation, and experience working with the elderly are also required.
