Social Worker, Care Management Monday-Friday, 8am-4:30pm Full Time Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge.

Key Responsibilities

• Coordinate psycho

• social support for patients during hospitalization

• Develop and implement personalized discharge plans

• Assess patient needs related to care coordination, resources, and services

Requirements

• Bachelor’s degree in Social Work required

• Master’s degree preferred Experience:

• Minimum two years in acute care, SNF, physician offices, or community health settings Licensure/Certification:

• LBSW license in Maryland required

Benefits & Compensation

• Salary: $62,899 - $94,390/year

• 401(k) with employer match.

Work Schedule

• Full-Time position.

• No weekends required.
