About the Role
Join Meritus Health as a Social Worker – M-F Schedule in United States. Social Worker, Care Management Monday-Friday, 8am-4:30pm Full Time Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge.
Key Responsibilities
- Coordinate psycho

Requirements
- Bachelor’s degree in Social Work required
- Master’s degree preferred Experience:
- Minimum two years in acute care, SNF, physician offices, or community health settings Licensure/Certification:
- LBSW license in Maryland required
