The physician will provide direct patient care to a senior-focused panel, addressing both physical health and social determinants of health. Responsibilities include conducting wellness visits, managing chronic conditions, and performing screenings within a collaborative team environment.
Candidates must hold a Doctor of Medicine or Osteopathic Medicine degree and be board certified or eligible in Geriatric, Family, or Internal Medicine. A valid, unrestricted medical license and DEA license are required, along with fluency in both Spanish and English.
