We are seeking a part time (20-30 hours, 2-3 days per week) provider for our Lafayette Center located in Franconia, NH.

The Advanced Practice Provider collaborates with the center leadership and medical directors to deliver primary and urgent care services to nursing facility residents. They are responsible for achieving clinical goals and improving health outcomes through compassionate, high-quality medical care.
Candidates must be a licensed Nurse Practitioner or Physician Assistant with a Master's degree in Nursing or an ARC-PA accredited degree. A minimum of two years of experience working with adult or geriatric populations is preferred.
