We're looking for a Full-Time Lead Physical Therapist to become the clinical leader of an outpatient home therapy program.
You'll provide meaningful one-on-one care while taking ownership of the therapy program, building relationships with patients and families, collaborating with referring providers and care teams, and helping create a standard of rehabilitation that truly improves lives.
Responsibilities
- Provide high-quality, one-on-one physical therapy to patients in their homes.
- Serve as the primary therapy leader.
- Build strong relationships with patients, families, caregivers, and referring providers.
- Help develop fall-prevention, mobility, wellness, and functional independence initiatives.
- Support and mentor other therapy team members as the program grows.
- Help establish clinical standards and best practices.
- Participate in interdisciplinary care planning.
- Serve as a trusted rehabilitation resource for patients and their support networks.
- Help grow a therapy program focused on exceptional outcomes rather than volume.
