Job description
PRN - Physical Therapist – Memory Care
Finally, a therapy job designed around people—not paperwork?
At Prava, we believe exceptional care happens when clinicians have the time to build relationships, think critically, and truly know their patients.
That means:
You're not expected to see 18–20 patients every day.
You're not racing from treatment to treatment.
You're not buried in paperwork after hours.
Instead...
You'll spend your day providing meaningful one-on-one care to persons living with dementia, becoming an embedded member of a senior living community, and using industry-leading AI technology that eliminates much of the administrative work that burns clinicians out.
This Isn't Traditional Therapy
Our therapists become trusted partners.
Families know your name.
Caregivers rely on your expertise.
Nurses seek your clinical judgment.
Community leaders value your partnership.
Residents recognize your kindness, trust your presence, and respond to the relationships you build every single day.
Less Paperwork. More Purpose.
Prava's AI-powered clinical platform automates much of the documentation and administrative work that traditionally steals hours from therapists.
More time connecting.
Less time clicking.
More clinical reasoning.
Less busy work.
That's the future of rehabilitation—and we're already there.
Why Memory Care?
Because nowhere else can a therapist make a greater impact.
You'll help residents:
Walk with greater confidence.
Prevent falls.
Maintain independence longer.
Stay engaged in meaningful daily activities.
Improve quality of life.
Support families and care staff.
Every day is different.
Every relationship matters.
Every win is personal.
New Graduate?
Ask us about ASCEND.
Our ASCEND Mentorship Program was built specifically for therapists who want more than just orientation.
You'll receive:
One-on-one mentorship from experienced clinicians.
Clinical coaching.
Specialized memory care education.
Leadership development.
Confidence treating complex geriatric populations.
A clear career pathway—not just a job.
We invest in clinicians because we believe great therapists are developed—not simply hired.
What You'll Love
Smaller daily caseloads focused on quality over quantity.
Meaningful one-on-one patient relationships.
AI-powered documentation and workflow support.
Embedded within one community—not bouncing between facilities.
Exceptional clinical autonomy.
Competitive compensation and benefits.
Continuing education support.
Opportunities to grow into Lead Therapist, Clinical Mentor, or Practice Leadership.
We're Looking For Someone Who...
Loves building relationships.
Wants to practice at the top of their license.
Believes every resident deserves exceptional rehabilitation.
Enjoys collaborating with interdisciplinary teams.
Is excited about shaping the future of therapy through innovation.
Our Promise
No double-booking.
No treating therapy like an assembly line.
Just exceptional clinicians, extraordinary technology, and enough time to provide the care your patients deserve.
In memory care, we're not trying to change a diagnosis.
We're helping people preserve their independence, protect their dignity, and create more moments of connection with the people they love.
If that mission speaks to you, we'd love to meet you.
Come help us redefine rehabilitation.
