What You'll Do
The RN Case Manager coordinates patient care from admission through discharge by collaborating with providers, healthcare teams, patients, families, and community partners to ensure safe, efficient, and patient-centered transitions. This role evaluates patient needs, develops discharge plans, and supports quality outcomes through effective care coordination and resource management.
* Coordinate interdisciplinary care plans, admissions, discharge planning, and transitions of care to support positive patient outcomes.
* Collaborate with providers, nursing staff, therapy, social services, outside agencies, and community resources to address patient needs and barriers to care.
* Assess patient needs, including social determinants of health, and connect patients and families with appropriate resources and support services.
* Monitor length of stay, discharge barriers, readmission risks, and opportunities to improve quality, efficiency, and continuity of care.
* Maintain accurate documentation, participate in care coordination rounds, and support compliance with hospital policies, regulatory requirements, and nursing standards.
* Performs a variety of tasks and other duties as assigned
Who We're Looking For
We are seeking an experienced Registered Nurse with strong clinical knowledge and care coordination skills to support patients throughout their healthcare journey. The ideal candidate is organized, collaborative, and committed to improving patient outcomes through effective communication, advocacy, and transition planning.
* Current Colorado Registered Nurse DORA license required; BSN or equivalent preferred, along with a current American Heart Association BLS certification.
* Minimum of 5 years of nursing experience preferred, with case management or discharge planning experience highly desired.
* Strong knowledge of professional nursing practices, patient care standards, safety principles, and healthcare regulations.
* Skilled in communication, collaboration, and building effective relationships with patients, families, providers, and interdisciplinary teams.
* Proficient in electronic health records, utilization review systems, Microsoft Office, and other computer applications used to support patient care.

Compensation may vary outside this range depending on a number of factors, including a candidate’s qualifications, skills, competencies and experience.
