The Hospital Social Worker is responsible for performing cross-continuum care coordination, psychosocial assessments, and interventions to improve patient outcomes. They provide customer service to patients and families while ensuring medically appropriate transitions of care.
Candidates must hold a Master’s degree in Social Work and be licensed in Washington state as an LICSW or LSWAIC. A minimum of three years of hospital or equivalent experience is required.
