This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge.

The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions. Additional Job Description Education: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education.
