Physical Therapist- Home Health Visits

Key Responsibilities

The Physical Therapist will perform home health visits to conduct diagnostic tests, develop individualized care plans, and provide rehabilitative interventions for adult and geriatric clients. They are responsible for educating clients and families on safety and health while accurately documenting care using electronic medical records.

Requirements

Candidates must hold a current Maryland Physical Therapist license and have graduated from an accredited physical therapy program. Strong interpersonal, organizational, and computer skills are required to manage independent home visits effectively.
