• The social worker will provide timely and appropriate discharge planning taking into consideration the patient’s individual goals, preferences, needs and resources.

• DEPARTMENT DESCRIPTION: UCSF Langley Porter Psychiatric Hospital – Mount Zion serves as a location for our inpatient, partial hospitalization and intensive outpatient programs.

• The UCSF Inpatient Program is a 30-bed mental health service caring for adults with urgent behavioral and emotional concerns who are referred by a mental health care provider.
