Key Responsibilities
The RN Case Manager is responsible for assessing patient conditions, developing individualized plans of care, and coordinating multi-disciplinary services. They provide direct skilled nursing care, supervise nurse aides and LPNs, and ensure all clinical documentation complies with Medicare and federal guidelines.
Requirements
Candidates must hold a current Registered Nurse license and have at least one year of experience in an acute care setting. A valid driver's license and auto insurance are required, with home care experience being highly desired.
