Key Responsibilities
The Registered Nurse Case Manager oversees patient care plans, performs clinical assessments, and coordinates multi-disciplinary services in a home health setting. They are responsible for accurate clinical documentation, patient education, and supervising nurse aides and LPNs to ensure quality care.
Requirements
Candidates must hold a current Registered Nurse license and have at least one year of experience in an acute care setting. A valid driver's license and auto insurance are required for travel to patient homes.
