Key Responsibilities

Perform initial and ongoing patient assessments, including OASIS documentation, and develop tailored treatment plans in collaboration with the care team. Provide patient and caregiver education while maintaining accurate clinical records and supervising physical therapy assistants.

Requirements

Requires a current physical therapy license, a valid BLS certification, and at least one year of experience. Candidates must have a valid driver's license and the ability to perform physical tasks such as walking, standing, and lifting patients.
