What You'll Do

As a PRN Physical Therapist, you'll help residents improve strength, balance, mobility, and functional independence through individualized, evidence-based rehabilitation.

Your Responsibilities Will Include

• Perform comprehensive physical therapy evaluations and assessments.
• Develop individualized treatment plans based on each resident's clinical needs and functional goals.
• Provide skilled physical therapy interventions to improve mobility, balance, strength, transfers, and safety.
• Monitor patient progress and adjust treatment plans as clinically appropriate.

Collaborate with Physical Therapist Assistants, nursing staff, and interdisciplinary team members to coordinate care.

Educate residents, families, and caregivers on mobility techniques, fall prevention, and safe functional activities.

Maintain accurate and timely clinical documentation in accordance with professional standards and applicable regulations.

Promote a positive, supportive, and patient-centered rehabilitation experience.

What We're Looking For

• Graduate of an accredited Physical Therapy program.
• Current, active Pennsylvania Physical Therapist license or eligibility for licensure.
• Strong clinical assessment, treatment planning, and communication skills.
• Ability to work independently while contributing to a collaborative rehabilitation team.
• Compassionate, dependable, and committed to providing excellent patient care.
• Skilled nursing facility or geriatric rehabilitation experience preferred.
