Job Description

- Must possess a valid Florida driver's License, valid auto insurance, and reliable transportation. Education and Experience:
- Minimum of one (2) year of inpatient nursing experience or previous hospice/home health and/or Long-term Care experience preferred.

- Standing: 66-100% of the shift
- Sitting: 0-33% of the shift
- Walking: 66-100% of the shift
- Lifting: 0-50 lbs. as needed
- Carrying: 0-25 lbs. as needed
- Pushing: 0-250 lbs. on wheels with assistance as needed
- Climbing: 1-5 flights of stairs as necessary
- Pulling: 0-250 lbs. on wheels with assistance as needed
- Bending: Proper bending as necessary to pick up things off the floor
- Squatting: As necessary while providing patient care
- Rotating: Shoulders as necessary
- Crawling: If necessary
- Kneeling: As necessary while providing patient care
- Reaching: Overhead while providing patient care
- Visual and hearing senses must be adequate to enter data in computers and mobile devices
- Utilize telephones and perform detailed paperwork.
Key Responsibilities
Evaluates and admits hospice patients, provides skilled nursing and symptom management, and coordinates individualized care plans with physicians and the interdisciplinary group. Supervises hospice aides and LPNs, maintains compliant documentation, and supports continuity and quality of care for patients and families.
Requirements
Requires a current Florida Registered Nurse license, a valid Florida driver's license, auto insurance, reliable transportation, and strong communication, organizational, and clinical judgment skills.
