Elara Caring is hiring a compassionate Physical Therapist to serve home health patients throughout Stamford and surrounding lower Fairfield County communities.
You will provide personalized therapy in the place where progress matters most, the patient’s home. You will have the independence to manage your visits while collaborating with an experienced clinical team focused on helping patients improve mobility, safety, and quality of life.

- What You’ll Do
- Evaluate patients’ mobility, strength, balance, functional abilities, and home safety needs
- Develop and implement individualized Plans of Care
- Provide hands-on therapy to improve mobility, independence, and quality of life
- Establish measurable treatment goals and monitor patient progress
- Adjust treatment plans based on changes in condition and functional outcomes
- Educate patients, caregivers, and family members on exercises, equipment, and safety
- Coordinate care with physicians, nurses, therapists, and other care team members
- Direct and support therapy assistants when applicable
- Complete accurate and timely evaluations, treatment notes, and clinical documentation
- Serve as a clinical resource and advocate for patients receiving therapy at home
- What You’ll Bring
- Graduate of an American Physical Therapy Association-approved physical therapy program
- Current and unrestricted Connecticut Physical Therapist license
- At least one year of experience in a healthcare setting
- Strong assessment, communication, organization, and documentation skills
- Ability to work independently in patients’ homes
- Willingness to travel throughout Stamford and the assigned coverage area
- Must be able and willing to travel 50%
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations

Key Responsibilities

Requirements
