This position offers weekends off. Work with Hospice of North Alabama in Double Springs, Alabama.
Key Responsibilities
The RN Case Manager will assess patient needs, create personalized hospice care plans, and provide direct nursing care including pain and symptom management. They will also collaborate with an interdisciplinary team and provide education and support to patients and their families.
Requirements
Candidates must be a graduate of an accredited nursing school with a current nursing license and at least one year of nursing experience. Experience in hospice or a similar setting is preferred, along with a valid driver's license and insured vehicle.
