Job Title: Social Worker

Location: Bloomington Hospital

Position Type: Full-Time Day, Monday-Friday 8:00 am - 4:30 pm 

Position Overview

The Social Worker meets face-to-face with patients and families to assess
psychosocial needs and provide support related to social support programs and
community resources. This role involves interviewing patients and relatives to
gather social history relevant to medical conditions and care planning. The
social worker assists patients in overcoming barriers that may impede their
ability to obtain maximum benefits from medical care, acting as a liaison
between medical, nursing, outside agencies, and patients/families. The goal is
to deliver solution-focused assistance tailored to each patient's medical and
hospitalization needs.

Key Responsibilities

Conduct face-to-face interviews with patients and families to assess social
history and psychosocial needs.  

Identify barriers to care and assist patients in accessing social support
programs and community resources.  

Serve as a liaison between healthcare providers, patients, families, and outside
agencies to facilitate effective communication and support.  

Provide solution-focused assistance related to medical conditions and
hospitalization.  

Collaborate with multidisciplinary teams to develop discharge plans and resource
development strategies.

Qualifications & Requirements

Education: Bachelor's Degree in Social Work (BSW) required.  

Experience: Minimum of 3 years in healthcare, social work, or related fields.  

Licensure

Permanent Indiana LBSW licensure required for all new hires effective October 5,
2025.  

Incumbents prior to this date must obtain their LBSW licensure within 6 months
of hire.  

Incumbents with their LSW are required to maintain licensure.

Skills and Knowledge

Strong assessment, resource development, and discharge planning skills.  

Excellent communication, planning, and collaboration skills within a
multidisciplinary team.  

Knowledge of state and county adult and child protective legal mandates.  

Understanding of growth and development principles across the lifespan.  

Experience in adapting services to diverse populations geographically,
culturally, and financially.  

Computer literacy and proficiency in documentation within electronic medical
records.
