Job Description: The Mental Health Therapist - Staff is an integral member of

the Primary Health Care Team and works in part of the primary network team in

the medical home model. As a member of a multidisciplinary health care team, the

position is primarily responsible for behavioral health supports for a number of

conditions under the care of the Primary Health Care Provider. The services to

be delivered through this position include screening functional assessment and

develops behavioral changes plans in conjunction with the Primary Health Care

Provider. The Mental Health Therapist Staff develops and implements a behavioral

healthcare plan and makes referrals as required.

Experience

* Experience w/ clinical interventions in Integrated Care or Community Health

Care

* Experience with clinical interventions related to Primary Health Care

* Experience in brief treatment modalities in PHC setting is an asset

Education

* Degree in Human Services (Social Work, Psychology, Registered Nurse)

Licenses

* Valid Class 5 driver's license

Key Responsibilities

The Mental Health Therapist provides behavioral health support and develops behavioral change plans for patients within a primary health care team. They are responsible for conducting functional assessments, making referrals, and implementing healthcare plans to improve patient functioning.

Requirements

Candidates must hold a degree in Human Services such as Social Work, Psychology, or Nursing and possess a valid Class 5 driver's license. Experience with clinical interventions in integrated or community health care settings is required.
