Key Responsibilities

Assess client home environment, factors impacting provision or barriers to client’s care.

Make recommendations for community and other resources for clients and family which facilitate the established plan of care.

Participate in client conferences as required.

Must be able to lift 50lbs.

Requirements

NY State LMSW license

2 years working as a social worker in a clinical environment.

Experience working in Home Care as a Social Worker preferred. Responsibilities:

Assess client home environment, factors impacting provision or barriers to client’s care.
