Nurse Navigator
Job Description
The Nurse Navigator serves as a consistent care coordinator ensuring timely
access and seamless coordination of services throughout the continuum of care,
from diagnosis through survivorship. The Nurse Navigator will meet with the
patient and their support systems to provide emotional support and guidance. As
a single point of contact, the Nurse Navigator will provide medical information,
education, and share knowledge regarding internal and external support services
and resources.
Department: Medical Oncology
Hours: Monday-Thursday 8am-4:30pm; Friday 8am-3:30pm
Required: ADN, Registered Nurse, BLS or CPR within 90 days of hire
Pay: Based on experience, starting at $35.43
Responsibilities
Provides education and information to the patient and family. Initiates and
documents patient teaching, including family and significant others, based on
assessment of needs., Relays information between provider(s) and client(s),
which may include but is not limited to communications with patients, families,
pharmacies, nursing homes, office staff, insurers, and facilities. At provider’s
instruction, communicates provider advice, instructions, orders and results, as
applicable, including accurate documentation of exchanges., Schedules patients
for diagnostic, therapeutic, referral, or additional office visits., Serves as
a resource for community educational events, such as health fairs, screenings,
symposiums, and lectures., Supports the patient during difficult
decision-making periods. Assists in coordination of end-of-life care for patient
and provides emotional support.
