Requirements

• Doctor of Physical Therapy (DPT) degree: Required; only if obtained license on or after January 2016; or foreign equivalent. Licenses/Certifications:

• Physical Therapy licensure in all applicable states of practice: Required

• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Required

• Valid Driver’s License (DL) and must be at least 21 years of age or older: Required Essential Functions:

• Salary: $95K - $143K

• PSLF eligible (Public Service Loan Forgiveness).
