The Medical Social Worker provides, directs, and evaluates medical social services for clients in a home environment to support their health and emotional well-being. Responsibilities include care coordination, discharge planning, and supervising Bachelor's level social workers.
Candidates must hold a Bachelor's degree in Social Work, Psychology, or a related field and possess at least one year of medical social work experience. A valid driver's license, good driving record, and the ability to pass a Level 2 background screening are required.
