Key Responsibilities

The Case Manager 3 manages behavioral health cases by developing and reviewing Individual Support Plans and conducting regular field visits to monitor member safety and progress. They also collaborate with multidisciplinary teams, mental health agencies, and psychiatric facilities to ensure appropriate service delivery and discharge planning.

Requirements

Candidates must possess a degree in social work, psychology, special education, or counseling, or be a registered nurse, along with at least one to two years of relevant case management experience. Additionally, applicants must obtain a Level One fingerprint clearance card and pass background checks including child and adult protective services registry searches.
