Key Responsibilities
The Case Manager will facilitate and write Individual Support Plans (ISPs) and conduct regular field visits to monitor the safety and progress of individuals with developmental disabilities. Requirements
Candidates must possess a degree in social work, psychology, special education, or counseling, or be a licensed registered nurse. Additionally, they must have at least one to two years of relevant case management experience and obtain a Level One fingerprint clearance card.
