Key Responsibilities
Provide comprehensive outpatient primary care to a senior patient population with complex chronic conditions. Work within a value-based care model alongside a dedicated care team to ensure continuity of care and positive patient outcomes.
Requirements
Requires an MD or DO degree with board certification or eligibility in Family Medicine or Internal Medicine. Candidates must have an active medical license, DEA registration, and at least two years of outpatient clinical experience.
