Key Responsibilities
The physician will provide outpatient primary care to a senior patient population with complex chronic conditions. Responsibilities include managing small patient panels, collaborating with a dedicated care team, and ensuring continuity of care.
Requirements
Candidates must hold an MD or DO degree and be board certified or eligible in Family or Internal Medicine. A minimum of 2 years of outpatient experience and an active, unrestricted medical license are required.
