Key Responsibilities
The physician will provide advanced primary and specialty care to geriatric and medically complex patients in their homes or care settings. They are also responsible for providing medical leadership, mentoring nurse practitioners and physician assistants, and coordinating care with interdisciplinary teams.
Requirements
Candidates must hold a Doctor of Medicine or Osteopathy degree with a current unrestricted state license and board certification in family practice, internal medicine, geriatric, or palliative care. Prior supervisory experience and reliable transportation for daily travel within the service area are required.
