Key Responsibilities

The Nurse Practitioner will provide advanced palliative care to patients in their homes, developing and implementing individualized care plans in collaboration with the medical director. They are responsible for performing in-depth patient assessments, managing treatment plans, and maintaining accurate clinical documentation.

Requirements

Candidates must hold a Master of Science in Nursing and have at least two years of Nurse Practitioner experience. A current RN license, Nurse Practitioner certification, and DEA license are required, with ACHPN certification preferred.
